Support Coordination

About the Program

Support Coordination is a service that assists individuals with disabilities, 21 years of age or older, in gaining access to programs and services that will help them achieve their hopes and dreams, and live a meaningful life.

JFS of Central NJ is a State of NJ Division of Developmental Disabilities (DDD) approved Support Coordination Agency.

Our Support Coordinators will assist individuals with Medicaid who are eligible for special services through the State of NJ DDD.
